The NCSHA Contest Raffle of a Samsung NU6900 Raffle is being conducted by Allita 360, a for profit organization, under the laws of the State of Ohio.

 

Entrants must be 18 years of age or older to participate and must have a valid Social Security number. Entrants are limited to one (1) entry.

Allita 360 members and their spouses are not eligible to participate in this raffle.

 

Raffle entries are only eligible with a completed survey and with valid contact information provided. Should the entrant's information prove to be invalid, their entry will be withdrawn from the drawing.
 

By IRS declaration, ticket purchases for valuable prizes are not eligible for tax deductions. Odds of winning are 1 in X, where X is the number of valid entries received.

 

Entries may be made in person at the NCSHA National Conference starting Sunday 10/20/2019 at 1:00 pm EST,  through Monday 10/21/2019 at 6:00 pm.

 

All entry forms must be received no later than 6PM on October 21st, 2019 or before all entry slips have been exhausted (1,000), whichever comes first, to be entered into the drawing. Allita 360 will not extend entrance beyond the current drawing date of 6:30 pm on October 21st, 2019.

 

Prize: One prize will be given away. First Prize: A Samsung NU6900 valued at $549.

Drawing will be held at Booth 35 of the NCSHA Boston conference. The winner will be determined by random drawing.

 

Winner need not be present to win. Winner will be notified by via phone or email the day of the drawing, and will be posted on the Allita 360 website.

 

The prize winner is solely responsible for all local, state and federal taxes, title, transfer fees, license duties, insurance and all expenses incurred in claiming the Samsung NU6900. The Internal Revenue Service requires that any prizes valued at $600 or more be issued a Form W-2G, and requires that 25% of total winnings be withheld for federal taxes on prizes of $5,000 or more. The First Prize winner understands, acknowledges, and agrees that the winner is receiving a prize and will hereby release and discharge Allita 360, and their employees, agents, officers, directors and legal representatives and assigns from any and all claims, liabilities, damages, losses or expenses arising from or caused by the award of such prize, including, but not limited to the payment of any and all taxes that are due.

 

Prize must be claimed within 30 days of drawing. An unclaimed prize after that date will constitute forfeiture, and no additional winners will be chosen.

 

Allita 360, its owner, and its agents assume no liability for lost, late, misdirected, mutilated, incomplete, illegible, or entries with invalid information which will be deemed invalid and disqualified.

The decisions and rules of Allita 360 are final on all matters concerning the raffle.
